If you are a current trust beneficiary and have a disbursement request, please submit via email at trustservices@futurecareplanning.org or by mail to:

A trust representative will review and reach out to you via phone or email with any questions regarding your request.

Trust deposits may still be mailed directly into our office or be set up as an automatic bill payment through your banking institution.

Tax Information

If you have a First Party Lump Sum or Third Party Lump Sum Trust account or if you are a recipient of gift funded by the Golisano Life Enhancement Fund, annual K1 statements are mailed to you. If you file taxes you may include this statement with your tax-filing documents. If you have questions regarding the K1 statement we encourage you to share the statement with your tax preparer.
