Process
Getting the help you and your loved one require is easy,
involving a four-step process. The staff at Future Care will be there
to assist you every step of the way.
Meeting 1:
Information about the program is provided, planning concerns are discussed and enrollment paperwork is completed. The family is provided with a list of documents that need to be provided at the next meeting. The second meeting is scheduled.
Meeting 2:
The family and the person with a disability meet with the Intake Specialist,
who facilitates a Letter of Intent Interview. The Letter of Intent,
though not a legal document, is a personal history of the person with
a disability from the caregiver’s perspective, as well as the perspective
of that person. It is meant to be the starting point for planning,
helping the family articulate the things that are important in ensuring
a life of quality for their loved one. We capture a person’s
day-to-day needs, hopes and dreams for the future and ask the family to
think about what they would want to leave as their “legacy of love.”
The third meeting is scheduled at this time.
Meeting 3:
The family meets with the Planning Specialist. Prior to this meeting the Planning Specialist has reviewed the Letter of Intent and all documents provided by the family. If appropriate, the consultant attorney has reviewed all legal documents and will attend this meeting if necessary. The Planning Specialist facilitates a conversation with the family about the specifics of the legal and financial situation, gathers relevant information and often advises families on next steps. The final meeting is scheduled at this time.
Meeting 4:
The family meets again with the Planning Specialist. The family receives an informational planning binder, copies of the comprehensive individualized plan and the Letter of Intent. The Plan is explained in detail and the family is provided with all available options, including referral resources and a plan of action. If the family so chooses, we can execute Health Care Proxies, Powers of Attorney and Living Wills for all of the family members.
Guardianship Proceedings:
In some cases, Guardianship proceedings are required. The consultant attorney attends the proceeding with all families who have filed in Monroe County. The attorney’s attendance in court in all other counties is determined on a case-by-case basis.

What to expect:
You will either reach a staff member immediately or you may have to leave a message. Someone will get back to you within 2 business days. You can expect to be scheduled 4 – 6 weeks from the time of your inquiry, sometimes sooner. The planning process, once in progress, takes about 3 months to complete.
